ENROLLMENT

 

Little Giants Early Education & Care is open to children from 3 months to age five.  Parents can apply for enrollment of their child by completing the following steps:

• Tour our program.

• Submit an Enrollment Packet (posted below):

  • ​Parent Handbook/Operational Policies Agreement Form (attached online)

    • Health History & Immunization Record 

    • Consent for Emergency Medical Treatment
    • ​Identification & Emergency Contact Information 
    • Nebulizer Care Consent Form (if applicable)

    • Affidavit Regarding Liability Insurance 

    • Parent Notification of Additional Children in Care 

    • Acknowledgement of Notification of Parent's Rights 

    • Infant (0-24 months) Individual Sleeping Plan (if applicable)

    • Food Program Enrollment Form (given at time of enrollment)​

    • Allergies Form (If applicable)
